1. 需要哪些疫苗
这是体检医生列的单子：
Proof of a MMR(measles, mumps & rubella) vaccine, Tdap vaccine, Varicella
vaccine( if you have not had the Chicken Pox before). You will also need to
have a RPR (syphlis) test and a Tuberculosis Skin Test within the past 3
months.
也就是说如果你有这些打过疫苗或者做过检查的proof，就不用再在医生那儿做，带上
证明就行。
2. 带上photo ID
3. 家庭医生处的打疫苗和检查insurance都cover. 移民医生处的不cover （我的不
cover,以你问过为准）。
4. TB test是需要2到3天的观察期的。到时要去复查，心里有个数。
5. MMR 和Varicella是可以通过blood test证明的。如果手里没有小黄本，做个blood
test就可以。
6. 体检医生没说flu shot.但是移民局要求上的确是说要这个if flu season.自己决定吧。

多谢，我也不知道我有没有过Chicken Pox，我的小黄本上是空的，是不是就应该打阿
？　是不是应该把RPR (syphlis) test and a　Tuberculosis Skin Test提前在自己医
生那里做了比较好啊？

我的小黄本上显示MMR要花两次完成，间隔一个月。不知道美国这边是不是一样。如果
你可以做血检顺便查一下，不用打当然好。
如果你的体检医生接受在其他地方的test result.当然做好了再去，我在家庭医生那儿做，至少insurance可以cover. 如果体检医生离你路远的话，也是在附近的家庭医生那儿做好啦。

MMR美国这边一次就行了。
但是水痘那个要打两针

儿做，至少insurance可以cover. 如果体检医生离你路远的话，也是在附近的家庭医生
那儿做好啦。

Blood test
You have to do blood test anyway for the RPR. So just add another test for
vaccine.

I also did an HIV test as part of the blood test, is it not required now?
ALSO, for those TB skin test shows positive, you can do the X-ray with an
outside radiology center which should be covered by your insurance.
I had pretty much everything covered by Insurance except the medical
examination.

请问个问题，Supplemental I-693就是Immunization Record吗？同一个东西？哪里找
到这张所谓的Supplemental I-693表？还是就是指的i-693表5页中的一部分而已
10. The I-693, Medical Examination, and/or Supplemental I-693, Immunization
Record, must be submitted unopened and in the sealed envelope from the civil
surgeon. The civil surgeon should provide a copy of these forms to the
applicant before sealing them in the envelope that is submitted with the I-
485 application.

如果有国内打过这些疫苗的纪录，可以作数吗？
另外，TB皮试后，过2，3天再去看以后，当场就能拿到体检报告吗？还是还得等几天？

Yes, if you have records of MMR,Tdap, Varicella, bring them with you.
As for the TB test, that depends. If it is negative, sure you can get the
report the same day. But if it is positive, you need to do X-Ray, which may
take days to get it done.
