1. 对于MSSQL，是不是只要是Index就是B-Tree structure？
2. Clustered Index 的 page 是单独存在，还是直接用表本身？
3. 不明白Key是干什么的，既然有Index这个概念，为什么还要有Key。

key is for data intergurity, make sure every row can be indentified by a
value
google the difference between primary key and unique key
index is for searching
