I had the same question and asked the new university. They replied me
as:
XXX,
You can either have a physician at your current school fill out
the form, or you can get a copy of your records, fill our form out yourself
and submit a copy of your records so we can verify and sign the form for
you.
Meanwhile,I also asked the health center at my current university, they told
me that:
Hi, XXX. I would encourage you to call our immunizations office
directly at XXX-XXX-4619. It may be that the Univ o
