I don't think there is standard format. But I have ever seen the similar
document. This of yours possibly is not good enough. Because,
(1) A certifies B, and then B certifies translations. Too complicated.
(2) IO must be convinced by facts for "fluent in both languages".
My 2 cents:
I am a professor in University of ***, my native language is Chinese; and I
have been living in U.S.A. for *** years. Accordingly, I am fluent in both
Chinese and English.
I certify that all of these translations are accurate.
