Part 1
1)I-94 number是I797上的，还是上次入境时白色i94上的？
2)Current nonimmigrant status(H1B) expires on___ (是填现在I797上的到期日么?
）
Part 3
1. I/we request that my/our current or requested status be extended
until___ (是填I20上的到期日吗？）
Part 4
3g. Have you, or any other person included in this application, been
employed in the united states since last or granted an extension or change
of status (填no吗？我现在在工作，但这是第一次转）
还有学校所在地址属于USCIS California管辖，自己居住地属于USCIS Vermont管辖，
是不是应该
file到Vermont?
