几个月前动的一个小手术，当时直接付了cash，后来file了claim，但是保险公司拒绝
付款，理由如下:
The modifier is missing, invalid or incorrect for the procedure billed.
请问这里的modifier指的是什么呢？3x!

modifying code of the procedure being billed
It should be straightened out between doctor's office & insurance company.
