To request the 17-Month STEM Extension from Immigration you will need to
complete the following forms:
Cover sheet
Optional Practical Training 17-Month OPT Extension Request form
o Please complete the top portion of this form prior to asking your Employer
to complete
the next section.
Completed Immigration Form I-765 (available at www.uscis.gov)
The answer for question #16 is: ( c )( 3 )( C )
Two (2) passport photos
o See specifications in the Form I-765 instructions. Photos may be purchased
at the
front desk in the Office of International Affairs
Copy of Form I-20, endorsed for the 17-Month STEM Extension by an
International
Student/Scholar Advisor
Copy of the STEM degree
Copy of your I-94 (both sides)
Copies of your former I-20’s if you have previously been approved
for any type of off-campus
employment
Copy of your passport (photo and identification page, expiration
date and visa page)
Check or money order for $380 written out to U.S. Department of
Homeland Security

offer letter不需要，但是规定opt失业后必须立刻汇报，因此最好当前的offer比现有
opt失效期要长点，这样申请延期期间没有失业的汇报，申请延期没啥问题。
但是如果opt批准前offer终止失业了，如何能满足延期opt规定的雇主e-verify要求，
结果就不知道了。
