hmo 需要 refer 吧。 ppo 不需要，但要从保险名单里选医生

going to hospital for labor belong to inpatient or outpatient, our HMO
requires $250/procedure outpatient deductible and $500/admission inpatient
deductible, what does this mean? Another silly question, if C-section fully
covered by HMO
