I have asked edd through their website for same questions
Here is their answer:
You need to reestablish the claim for the remaining time via email or phone
1-877-238-4373
You need to request a reestablished claim form if you are not able to get
through the phone lines is via email. You can email the department about a
week or 2 before the dates you will be taking off and request one.

好像那个form是问第一次申请的假期结束后，要不要继续使用剩余的假期。因为我休完
PFL三周后，去上班了，就没管那个表。一会再去找出来研究一下 :-)
