1: how to answer: Enter for the most recent I-94 card in your passport, even
if expired.
I entered USA last time under F1 Visa. The I-94 shows D/S. How to answer
the question” date of expiration of I-94”? I filled with “F1” on “the
immigration status on I-94”, but I used the expiration date of extended H1B
(there is an unused I-94 attached) , is this right? Same problem with my
wife.

The problem with my son is different. His last I-94 on his passport shows “
I-94 number is XXXXXX” and it shows expiration date “XXXX” ( I filled
with this number and date??). This I-94 number is the same as that in his
first H4 approval notice, but it is different from that on his extended H4
approval notice (XXXX)
---Use the date on the extended H4 notice
2: my wife applied three times EAD (F1 OPT applications and all of them were
approved. I only filled her most recent EAD information (they are same “A#
” number, but different application date). Is that ok?
---No, she doesn't have A#, leave it blank
3: I filled an “A#” from my EAD card (It is from F1 OPT application.). BUT
THIS A# IS DIFFERENT FROM THE A# IN THE APPORVED 140 FORM. Is this fine?
---It should be the number on the 140 approval notice.
4: when we filled our information such as address for the medical
examination, three of us used same address (my wife’s). is that ok? Three
of us live in three locations.
---I don't think it matters.
5: when they ask about previous working, living history of LAST FIVE YEARS,
I put information starting from Jan 2007 (2007, 2008, 2009, 2010, 2011). Is
that ok?
6: My son’s information about City of residence and country of residence (
he was born in china, but now study in a University here) I filled with his
current city and “ United States” respectively, is that right?
