We are preparing for documents of I-485, this is the list we got from the
lawyer, hope this is helpful and hope we can get I-485 approved soon ~~~~~
Copies are fine unless otherwise indicated and should be on 8 ½ by 11
paper, one-sided, and unstapled.
1. I-140 Approval Notice
2. Names, phone numbers, and e-mails of university contacts for
application (e.g. Dept. Chair or unit director, HR manager and/or other
relevant administration staff)
3. Letter from department or unit confirming continued employment
4. I-94 cards and passport I.D. and visa pages of alien and any
dependents
NOTE: Clear color copies of picture ID documents and photo pages (ID and
visa) from passport are preferred.
5. Any documentation on past immigration history
Please include copies of approval and receipt notices for alien and any
dependents; petition and/or application copies (e.g. copies of H-1b petition
packages).
6. Birth dates, birth places (town, province, country), and social
security numbers for alien and all dependents (including U.S. citizen spouse
or children)
7. Current Home Address & Phone Number
8. Foreign Address and phone number (or address abroad that may be used
if consular processing is necessary)
9. Birth and marriage certificates for alien and all dependents (if
either spouse has been divorced, also provide marriage certificates and
divorce paperwork for previous marriages)
NOTE: requirements for birth certificates are very strict. They must have
date and place of birth and both parents’ full names. If this is not
available, speak with Legal Affairs about alternative evidence.
10. Port of entry for last entry to United States (where you went through
immigration).
11. Completed medical exams (The exams must be done by a government
designated civil surgeon. A
list is available on the USCIS website. Be sure to ask if the doctor if he
has all necessary forms in his office!)
12. Immigration photos. Now the same as passport photos. (The number
depends on the applications being filed, but I would recommend six of each
person.)
13. Completed G-325A (provided by attorney -- ask for help if you need it
!)
