Make appointment with family and immigration doctors, and take the exams.
The tricky part is that you have to do TB-test, even your X-ray is clear.

Possible reasons for 485 REF (reviewed from online resources)
1. Birth certificate (because it is late registered / not having both
parents name clearly / language translation issues ...)
2. Medicals - You should have indicated the skin test reaction then X-Ray,
immunization records, checkbox 1 or 3 ticked in supplemental I693).
3. Marriage certificate or other marriage related documents.
4. I-94, Visas, copies of passport.
5. Employment verification letter (but that would not be sent to dependent.)
6. RFE的内容是指纹两次不过, 需要最近5年的居住地PD出的cert of good conduct
7. 签名 不清
