这个时候收到这张1.5% cb无年费卡，估计是把这个当成FIA卡的替代品吧？
这个应该不算AMEX自己发行的卡，不计入四张卡的限额，对吧？如果serve还活着的话
，可以用它online loading赚1.5% cb么？

Hi all,
Last week in order to phase out CVS i purchased a Bayer coutour meter to get
the rebate. But tonight I opened the box and there is no rebate form. Could
someone tell me where to find the form? Or can I return it to CVS? Had no
experience on this issue. Thank you very much.

你买的盒子有没有写rebate字样？如果没有，估计是新版，新版的没有试纸，没有
rebate form。你都打开了，估计要退货很难了。
