给保险公司打了好多电话，有的说只要是in-network durable medical equipment
supplier提供的，保险100% cover。有的说cover上限$150。
给supplier打电话，要我签个waiver form，说万一保险不报，我自己掏腰包每月$170
。。。这不比自己买还贵吗？
想问问有没有MM成功拿到奶泵的？都是什么牌子啊？产后要回去上班算不算medical
necessity？
查了一下policy上这么说：
Lactation management aids are a covered benefit for Anthem Blue Cross State
Sponsored Business members. Hand held breast pumps can be obtained via
prescription without prior authorization.
Electric breast pumps are available for medical necessity for Anthem Blue
Cross State Sponsored Business members with a provider referral and prior
authorization. Contact the Utilization Management Department at (888) 831-
2246.
