我已经帮忙联系了波士顿儿童医院，院方可以对病情进行评估，远程诊疗。现在需要把
一些国内的诊断书和病历翻译成英文。我自己翻译很困难，而且不准确，医学术语太多
。所以想请版上好心人帮忙翻译，家人都很焦虑，如能相助，不胜感激。
另外，不知有没有人知道，还有哪里对Gorham disease有诊疗经验，国内对这个病了解
很少，没有任何诊疗办法。请有了解的人能提供一些信息，非常感谢！

想把电视挂在壁炉上方。要新接个outlet在壁炉上方，这样电视挂上后，看不见电源线
了。墙都已经打开了，忽然想起，不知道有没有安全规定说壁炉上方的墙上不能装电源。
第二个问题是，现在壁炉里有个小火苗一直燃着，这倒没啥，就是壁炉正面的玻璃很
热，大夏天的，岂不是更热。怎么能把这个关掉，然后冬天的时候再开。

History of Hospitalization: Complain left shoulder scapular pain, difficulty
in lifting in left arm for 5 years, chest tightness and short of breath for
2 months. Admitted.
Physical exam: Alert, oriented,calm. BP 120/80, left shoulder size < right
shoulder, left back ribs and left shoulder scapular indistinct. Normal rib
spaces.Normal breathing. No tenderness on chest. The left chest showed
dullness on percussion. Diminished left lung sounds on
inspiration.No rhonchi bilaterally.Wider border of cardiac dullness。 HR 80/
min. Normal SR. No elevated P2。 No murmur and friction rubs. Abdomen flat
and soft. No abdominal tenderness and rebound pain. No abdominal wall
varicosis.No palpable liver and spleen under rib cage. Negative Murphy's
sign. Normal bowel sounds X 4 quads. No edema on both lower extremities.

Labs：
Urinalysis(June 20): calcium oxalate crystals ++ in the urine. No other
abnormality. CBC, stool test, liver function, renal function, electrolytes (
June 20, 2012) are all within normal limits.
Bone metabolic index: osteocalcin 26.04ng/ml
Pleural fluid analysis: Pleural albumin--2.9mg/dl. Total pleural protein: 4.
7mg/dl.Chloride 108 mmol/L. Glucose: 5.3mmol/L.
Rivalta test： positive +++. Color: sanguinous. lymphocyte: 87%. Total
nucleated cell count: 6560.0 (X10^6/L). Acid-fast bacilli were not
identified in the pleural fluid. ADA, Neopterin/cellular immunity, pleural
tumor marker are all normal.
Chest Hydrocele Desquamate Cell Study：No malignant cell found.
Bone metabolic index: acid phosphatase： 2.11U/L, Bone Alkaline Phosphatase
： 15.5ug/L. CTX-Ⅰ： 1.54. 25-OHD：38.07 mol/L.
Sputum culture study: Negative.
CBC（July 6): WBC-6.8X 10^9/L. neutrophil-83.4%.
Liver function (July 6)- Alnumin-2.97mg/dl, ALT-28U/L,AST-20U/L.Creatinine-
89umol/L, Potassium- 3.47 mmol/L.

Pelvic x-ray: No abnormality.
Bone density: decreased
Chest X-ray: 1. change in left scapulashoulder, clavicle and ribs (bone loss
in left scapular and 7th-9th ribs). With clinical evidence, possible
diagnosis--Gorham disease. 2.high-density masses in left lungs. Left pleural
effusion. left atelectasis. 3. No abnormality on right scapulashoulder.
Skull X-ray:No abnormality.
B-Ultrasound：left pleural effusion (echolucent areas between ribs 6th-11th.
Poor sound transmission。 The most Depth： 168mm, 18 mm depth under skin.
Defined.
CT Result: 1.Less density in left scapulashoulder and 7th-9th ribs. With
clinical evidence, suspected diagnosis- Gorham disease. 2. Left pleural
effusion. left atelectasis.
Bone marrow hemocytology：Significant myelosis。trilineage yelodysplasia .
Platelet present.
History of Hospitalization：Comprehensive exams/tests after admission.
Pamidronate IV 60mg X 3 days. Good response. Pt. is stable now and
discharged home.To be followed up.
Pathology: Bone marrow hemocytology：Significant myelosis。trilineage
yelodysplasia . Platelet present. Chest Hydrocele Desquamate Cell Study：No
malignant cell found.
Diagnosis on discharge: Gorham disease， Left pleural effusion.
