You submitted the notarial certificate without the parents names. The U.S.
Department of State indicates birth certificates are readily available.
Submit a copy of the Household or Village Registry reflecting the entry of
the child and a copy of the booklet issued by the province's health
department reflecting the child's name, gender, date of birth, and both
parents names.
