12-29-2010 male 55, surgery to remove gallbladder and right-side liver with
5x6 primary tumor (location section VII). Adenocarcinoma, moderately
differentiated. Pathology confirms cholangiocarcinoma, intrahepatic origin.
No post-op chemotherapy
12-02-2011 multiple density-contrasted foci in liver, enlarged lymph node at
liver portal and veins, enlarged spleen. Multiple small cysts in liver,
with calcification. Small cysts on both kidneys. Small nodules in upper left
lung lobe. Expansion of bililary tubes inside liver, with wall thickening
of main duct. Enlarged lymph nodes at posterior peritonium. Conclusion:
recurrence of hepatic chlangiocarcinoma with local and distant metastasis to
liver, kidney, abdomen and chest.
Treatment after recurrence
- RFA to remove bile stones
- Transmetil injection to alleviate jaundice and itching
- so far, no chemo, no radiation
His onclologist in China recommended against chemotherapy, on grounds that
they are ineffective.
So, please from your experience
1. Chemo or no Chemo? If so, what chemo agents to try?
2. Tarceva?
3. Alternative?
