US now has a standard about hospital inforamtion and image databases, called
DICOM. You may get some hint from that.

Yeah,I know DICOM, but it concerns more detail.
I'm just thinking about the structure of the whole system.
Since this system will be used in many hospitals,I think it may be
something like a specific middleware.But I don't know how to get it.

Application organized by function units when connected through patient
record number.
1, Outpatient Department
2, Inpatient Department
3, Emergency Department
4, Pharmacy Department
5, Radiaology Department
....
It will be a huge project. Don't even think about to make a standard
model that every hospital will be willing to use it.
