When you guys ask for reference letter for GC, do you usually get the letter
with the institution's letter head (logo) on it? Or it's just a plain paper
with your reference letter? Thanks for your help!

yes with letterhead, 除非推荐人公司有policy不让私人用letterhead

one more question. Should I ask them to copy and paste my draft into their
letter head or they provide me a template and I do the copy paste? I am just
worried that the format will be changed during this process. Thanks!

they just add the letterhead to your draft. Just like head and foot
