从加拿大入境，安检在加拿大，US这边没有安检。请问place of last entry into the united states应该填那里？先谢过了！

you must got it in USA entry port, not Canada. For example, Buffalo, ..

I did a search for USA port of entry and found this link.

It looks like that there are some entries are not in US but denote as PFI (
preflight inspection). I am still confused which city should I use to fill
in the form, the canada one or the US one?

yes, an abbreviation of the canada airport.

还有我当时去加拿大只是去开会，并没有办签证，nonimmigrant visa number我是不是
应该用原来在国内签的啊？那个consulate where visa was issued and date visa
issued是不是也是国内签的时间和地点？还是都是加拿大的？问题比较多啊，我看了

我仔细比较了我现在的I－94和以前的I－94，不同的地方是以前的类别是F－1，D/S, Dallas, 现在的是H－1B，具体日期，VCV。
这个版上有没有直接从下边这些地方回来以后直接申请绿卡填写I485的，请大家帮忙参考一下。
