DS 3035 表有这个问题：
Date and place of first entry into the U.S. on your original exchange
visitor (J-1) visa. Entry information should refer to the first time the J-1
visa was used to enter the U.S. (If the EV changed to J-1 visa status while
already in the U.S., enter the date of status change, control number and
issuing post of that first J-1 visa.)
我是在美国换签证， change of status，所以要按它说的填写C/S日期，control
number和issuing post。但是我换的时候就拿了一张USCIS的approval notice，上面都
没有control number或者issuing post的。请问这些应该怎么填写？ 谢谢！！！！
