I would say add 0.125% to the best rate for 350K+. For the restriction of
how long do you have to wait to do another refinance, please consult with
your previous loan broker, usually there is a 4-6 months restriciton.

It varies due to the loan amount. Anything around 1% of your loan amount is
not fishy.
