Do I need to purchase Owner's Title Insurance again if I want to refinance?
Or previous Owner's Title Insurance is still valid even after refinace?
If I need to purchase the insurance again, should mortgage company cover for
that if they claim it is no-cost refinance?

You only need a new lender's title insurance. If this is no cost refinance,
it shall be 100% covered.
