1。checklist置顶的绿卡申请流程里有。用HH7网友的就行
2。全部都可以用复印件。

据我了解，134只用于非移民签证申请，864是用于移民申请，你可以看一下864表的
intro
普通EB类移民是不需要file 864的，只有family based和EB中几种特殊情况需要。

这是我作为H4副申请人的cover letter包括checklist。今年夏天拿的卡。暂时也是带
娃大妈。如果娃是美国生的，birth certificate上的父母名字是婚姻事实的最好证明
。跟老公同校同系，也就附了学位。没附银行证明和税表也过了。lz把F和H改改应该能
直接用。
