as soon as possible:
1. Your first & last name
2. Your mailing address (note: we ship via UPS so PO Boxes will not
be accepted)
3. Your phone number (for shipping purposes)
4. An Affidavit of Eligibility and Release (attached), notarized
5. Signed and completed W9 form (attached)

注意w-9，这个是要添SSN的，我记得好象。
打公司的general phone line 问，而且要查你收到的email address. Be careful!
The name of the person may be right, but this email address and phone # may
