Presenting complaint: Waxing and waning fever, hind limb paresis
History: Meebo is a 13 week old female spayed Domestic shorthair cat that
presented to the Texas A&M emergency service for fever and hind limb paresis
. Meebo was adopted about a month ago from the shelter. On October 5, she
was taken to Dr. Bostick to be spayed. That same day the owners noticed that
she was sneezing and had a small amount of diarrhea. She was prescribed an
antibiotic (Clavamox) and the sneezing seemed to stop. Around October 12 the
owners noticed that she was having trouble walking and seemed painful in
the hind end. She also developed a fever around the same time. Meebo was
taken back to Dr. Bostick where she received a single dose of Metacam and
was prescribed Clindamycin and Doxycycline. Dr. Bostick also did bloodwork
and took radiographs of the right hind limb. He also performed two FeLV and
FIV snap ELISA tests which were both negative. Ms. Glass reported that the
only vaccinations that she is aware that Meebo has received were Rabies and
FVRCP around September 22, 2011. Meebo has continued to eat well but she is
drinking less

water than she did before the spay. She has had intermittent bouts of
diarrhea over the last couple of weeks, but they have not seen any blood or
mucus in it. Meebo's medical record from Dr. Bostick was printed and put
into the chart folder.
Physical Examination: weight - 1.92 kg. T - 103.1. P - 210bpm. RR - 80bpm.
Mucous membranes pink/moist. CRT < 2 seconds.
Meebo was bright, alert, and responsive upon presentation. On auscultation,
Meebo's heart rhythm was regular and strong. No murmurs were ausculted. Her
lungs sounded clear bilaterally with no abnormal lung sounds. Abdominal
palpation was difficult to assess as her abdomen was tense. Meebo did
defecate in the exam room while we
were watching her walk and it appeared as if she did not know that she was
defecating.
Neurological findings - There was ataxia in both hind limbs as well as
possible pain on palpation of the lumbar spine. Muscle atrophy was present
in both hind limbs. Hopping deficits in both hind limbs were noted as well
as possible hopping deficits in both front limbs.
Diagnostic Tests & Results:
Indirect blood pressure (Doppler # 1 cuff on right rear leg) = Systolic 105;
95; 95
SpO2 : 96%
Thoracic and abdominal radiographs: No significant findings
CBC: Increased white blood cell count (23,000), increased protein level (7.3
).
Serum chemistry test results: Increased globulin levels, increased lactate,
increased
glucose, increased phosphorus levels.
Diagnosis: Open, suspect possible feline infectious peritonitis (FIP)
Instructions to Owner
Medications: continue antibiotics and pain medication as previously
prescribed until Meebo's appointment with Neurology Service on Monday.
Call If:
• Meebo's weakness suddenly becomes worse or she can not walk.
• Meebo becomes lethargic, stops eating, or stops drinking.
• Meebo appears to be in pain.
Additional Instructions:
At this time it is difficult to determine the exact cause of Meebo's
neurological signs. As we discussed today, there are many potential
infectious agents that could cause signs similar to this. These include the
dry form of Feline Infectious Peritonitis (FIP), Feline Leukemia Virus (FELV
), Feline Immunodeficiency Virus (FIV), Toxoplasmosis, and several other
less common diseases. Some of Meebo's clinical signs and bloodwork findings
including her age, her neurologic signs, and the increase in her globulin
levels, point towards FIP. As we discussed, there is no definitive test for
FIP as it is a mutated form of corona virus which is commonly found in
healthy cats and kittens. There are also some non-infectious diseases such
as intervertebral disc disease or thiamine deficiency that could be causing
these clinical signs although these would be less likely in a kitten.
We recommend that you set up an appointment through the Texas A&M Neurology
service to have Meebo evaluated by them. As we discussed today, blood will
be drawn in order to submit for corona virus titers as well as any other
tests reccommended by Neurology. Please do not allow Meebo access to food
after 10pm Sunday night as sedation may be needed during her appointment on
Monday.
