What is messenger service for?
What will be effected if it is stopped?
(MSN IM? Outlook? SMTP? )

This is a service on Win2k/NT. It doesn't have anything to do with MSN IM or
SMTP. It allows you to send the message to some or all users in the local
domain, thru NetBios. Windows also uses it for something like a warning window
from the domain controller. By the way, some spam uses this technology too,
for example, the diplomat spam I saw before.

Is this stop it for once, or forever?

Once.
net stop/start command only start/stop a service for the current session. To
disable it forever, you need to Control Panel-> Administrative Tools ->
Service -> Messenger -> choose Disable from the 'startup type' box
