you can't input text on certain place since
the PDF file was made so that you can't edit
it using whatever software. but you can
circumvent this by first printing it to a
ps file then distill it back to a pdf.
if you have Acrobat as you said, you should be albe
to create form field at any location you want.
form tool icon is the 3rd one from bottom up
on the toolbar. or you can go to tools-->forms.
just follow themenu instructions.
