I have the answer for the A number field in 485 now: it's the A number
listed on your approved 140 (under beneficiary line). Don't use the one on
your OPT card.
Also if you didn't get the A number filled in your 693 (health exam), it's
ok. You don't have to redo that.

actually it does not really matter. I have sever different A number, of
course the USCIS issued. But the 485 receipt issued me a new A#, then last
week, when the greencard came, it is the A# on 485 receipt. So it is not
your business I guess.
