My wife and I had a short visit to Canada (<30 days) last month. This means
that our last entry place to USA was the border city of Canada. When we
drove to Canada, we did not give out I-94. When we drove back to the US, my
wife got a new I94 card for her H4 because her old I94 had something wrong
with the expired date. But I did nothing and just used USCIS approved H1B
form to enter it. In this case, how to fill out the following items of Part
3A:
Nonimmigrant visa number
Consulate where visa was issued
Date visa issued (mm/dd/yy)
