I visited Canada for one week last year. I only have a Canadian Visa stamp
on the passport without any other record. Should I use the date that I left
Canada and entered U.S. as "last entry" or use the date that can be found on
my passport (back in 2006)?

I have two I-94.
From 2003 to 2008, I was F-1 student. I visited China in 2006 and got an I-
94.
I changed to H1B in 2008 without leaving the U.S. and got a new I-94
together with I-797.
Do you suggest me to use the entry date on the old I-94? Again, there is no
record on the new I-94 when I enter the U.S. from Canada in 2010.

in my opinion, you should use the old I-94 when you were still a student
for the I-94 on H1b, you never left US with it (the number should be the
same
with the one when you were a student)
you entered U.S. from Canada, but you don't have a new I-94 for that

Yes, you're right. The two I-94 actually have the same number, which I have
never realized.
