I don't know for sure, but here's the explanation on USCIS website, and Your
case seems fine.

Civil surgeons completing medical examinations between Nov. 1 and Dec. 31,
2011, should use the current form dated 10/11/11. However, USCIS will not
reject the previous version of the form, dated 7/20/10, for this time frame.
Starting Jan. 1, 2012, civil surgeons MUST use the current form, or face
rejection by USCIS, requiring the applicant to return to the civil surgeon
and resubmit the current form.
