使用中国银行网上汇款到美国账户，提交后出现如下信息：
”汇款人地址（英文或拼音）不符合要求格式“
试了几次稍有不同的（国内）地址拼音都是这个错误提示。
请问有经验的大侠，应该是个什么格式？多谢

Will go to the civil surgeon's clinic again in early afternoon today.
Before going, just wanna double check with people here. Are MMR, Varicella,
& TD the only required shots? For other shots, should the doctor just
check "not age appropriate"?
MMR - LG and I both have records in our yellow vaccination documents from
China; does the vaccine require to receive in every XX years, just like Td?
Td - I will definitely get a shot today though the civil surgeon made a
mistake by putting a recent date in the chart. LG already got the shot
yesterday.
Varicella - LG had chickenpox when he was little. I never received a vaccine
and had that before. So is this one required? if yes, i would get it today
as well. And another question is "is that ok for me to receive two shots (
Td & Varicella) in the same day?"

报告一起发给移民局。

We went yesterday to check PPD and then my lg got the Td shot. We came back
and noticed some info was wrong about my Td date (i didn't have Td shot and
they put a 5/12/2011 in the chart), that's why we are going to doctor's
office again today.

About MMR, do we have confirmed answer about the valid time period? We don't
have time to do blood test :(
Also, just called the civil surgeon and was told they do not accept
insurance even though they are in PPO network. I guess that may be because
not filing with insurance they can charge a higher price - they charge $90
for Td and $160 for varicella. Are these reasonable?

Doctor put ”not age appropriate” in varicella. My yellow pamphlet doesnt
have a record for varicella.
I did ask the nurse and she said the is no time constrain on my mmr , but
considering other mistake they had, would like to do some homework before
going there today.

Thank you for the reply. we just have a couple of civil surgeons nearby, so
there is no competition in the market. Anyway, still happy that we finally
got this done and finger crossed on that our results are error-free:)
