insurance is much more expensive than one physical exam, which could be just couple of hundreds with the exam and tests (blood, urine, pap), while insurance normally run around thousand a month.

That is not true. We are buying insurance for our nanny. It's usually around $150-200 per months. But it's not a very good plan, there's like $50 copay with $2500 annual deductible or something like that. It also depends on your age and preconditions. On the other hand, if you pay for the tests yourself, you might get a cash discount. You need to talk to some insurance companies and local health providers to get a better sense.
