今天看自己的保险手册，有两个不明白的地方：
1、Annual Out-of-Pocket Maximums: $3000/insured person/year
2、Lifetime Maximum per condition: $250,000
究竟哪一个是保险公司能cover的最高限额，原来以为是第一个，但不知道out-of-
pocket指的是什么。是不是和non-PPO那部分有关？

250,000 is the amount that the insurance company is willing to pay for one
condition.
out-of-pocket is the amount that you have to pay.

如果out-of-pocket是我have to pay，为什么还会有maximum的限制？

that means they will make sure you won't pay above that amount in one year,
it's like a security net for you

the maximum out-of-pocket means once you've paid this
much with your money (combined deductibles, co-pays,
co-insurance, or whatever), the insurance will cover
100%, until the maximum coverage, or end of year.
