宝宝刚三周, 今天一天,哭闹不止, 吃液体奶加一点母乳.
吃液体奶的时候,有几次,有想吃又恶心的症状,所以吃吃停停.
母乳少,边吃边睡,一放到床上,便大哭. 中间翻身醒来也大哭,又要哄半天才睡.
今天从早上八点,到下午八点,总共睡了一个半小时. 我都快崩溃了.
求助大家, 不知道是不是colic的症状? 还有可能是什么问题? 只有我一个没经验的新
妈自己在家带宝宝,看宝宝痛苦的样子好心焦.
宝宝哭的样子和声音都和往常不同,不是"哇哇" 的声音,而是大人那种痛苦的声音和表
情.
给宝宝换了液体奶的牌子四天了. 这两天每天大便多次 (前面没换奶的时候,多数每天
拉一次),喂奶的时候,肚子咕噜咕噜响,大便也这样响,"哗啦"一声冲出来. 不知道是不
是拉肚子?

What are the Symptoms of Colic?
It is not uncommon for newborn babies to go through periods when they appear
abnormally irritable or seemingly cry for no reason. However, if you
suspect your baby is suffering from colic, you may look for the following
symptoms:
cries vigorously for long periods, despite efforts to console
symptoms occur around the same time each day or night, often after meal
times, and usually ending as abruptly as they began
shows signs of gas discomfort and abdominal bloating
has a hard, distended stomach, with knees pulled to the chest, clenched
fists, flailing arms and legs, and an arched back
experiences frequent sleeplessness, irritability and fussiness
In most cases, colic is the worst pain a baby has thus experienced. It is
usually manifested as an acute abdominal pain with intense spasmodic
cramping, but since colicky babies cannot describe exactly what distresses
them, it is hard for parents to know the precise cause of their distress.
Infantile colic is most common in the first few weeks to four months of an
infant's life; rarely does it endure past six months of age. Pediatricians
often use the "Rule of Three" to diagnose colic: "A baby that cries for
three or more hours per day, at least three times per week, within a three
month period". Wess, et al., "Paroxysmas fussing in infancy." Pediatrics
1984:74:998. About 25 percent of babies worldwide meet the official "Rule of
Threes" criteria for medical diagnosis of colic.
What Causes Infants to become Colicky?
There is no single consistent cause for colic that experts all agree upon.
However, a lot of evidence suggests that colic may be caused in different
ways in different babies. There appear to be several contributing factors
that, when occurring in combination, are likely to result in colic pain and
discomfort:
Newborns have an immature digestive system that has never processed food.
The gastrointestinal system is literally just learning to function. Muscles
that support digestion have not developed the proper rhythm for moving food
efficiently thought the digestive tract. Furthermore, newborns lack the
benevolent bacterial flora (probiotics) that develop over time to aid
digestion. This explains why almost all infants outgrow colic within the
first six months
Certain foods eaten by lactating mothers contain volatile chemicals and
allergens that in a small percentage of infants result in colic discomfort
and digestive upset. Through lactation, trace elements of cruciferous
vegetables and other gas producing foods may be passed via breast milk to
baby and cause gas and bloating.
Infants often swallow air while feeding or during strenuous crying, which
increases gas and bloating, further adding to their discomfort.
Since infants nervous systems are so immature, it is possible for them to
get overloaded with unfamiliar sights and sounds. Infants that are easily
overloaded often experience more severe colic, fussiness, and difficulty
sleeping later in the day or at night. In general, the more activity (
errands, visitors, T.V., phones, etc.) in baby's day, the higher the chances
of baby becoming colicky and fussy.
What Treatment Options are Available to Relieve Baby Colic?
Colic, and the months of distress and sleeplessness that it brings to both
infants and parents, can leave you feeling frantic, frustrated, worried,
exhausted, confused, guilty and inadequate. Foremost, it is essential to
build and maintain a loving bond with your infant. If your baby experiences
colic that does not appear to dissipate, you will probably be looking for a
safe, natural and effective colic remedy to ease your baby's suffering.
There are several treatments available for baby colic. As always, you should
consult your pediatrician first before giving baby any medications,
remedies or supplements.
Homeopathic remedies can be an excellent choice for treating infant colic
symptoms. Homeopathy is safe and completely allergen-free and side-effect-

cause over-alkalinity and exacerbate a colicky condition. Furthermore,
sodium bicarbonate is also absorbed into the bloodstream and thus can have
unwanted side effects. Studies have shown that sodium bicarbonate can
deplete and interfere with Folic Acid and Iron, indicating that it may
affect the function or absorption of both.
For this reason, sodium bicarbonate containing products are not to be
consumed by “children under 5 years of age”, as stated clearly on antacid
and baking soda boxes. Antacids always warn against extended use: “Do not
use for more than 2 weeks”. According to some doctors, sodium bicarbonate
can cause an imbalance in babies’ electrolytes, which can also lead to
serious problems.
Please note that antacids do interact with or prevent the absorption of many
medications including, but not limited to, aspirin, tetracycline,
penicillamine and ulcer medications (Zantac, Pepcid, Axid, Tagamet).
According to Medline Plus, a service of the National Library of Medicine,
antacids (sodium bicarbonate) “should not be given to young children (up to
6 years of age) unless prescribed by their doctor”. Since children often
cannot describe their symptoms well, doctor visits are advisable before
giving any antacids to children. The condition may require other treatment.
In such cases, not only will antacids not help, they may actually lead to
unwanted side effects and/ or worsening of condition.
Medline also warns against taking sodium bicarbonate with large amounts of
milk or milk products. To do so may increase the chance of side effects.
Essential Oils and extracts (dill oil, clove oil, fennel extract, ginger
extract, etc) are used by all other gripe water brands. There are known
safety issues with herbal oils and extracts. Many experts believe they are
not to be taken internally by adults, let alone infants. Please be well
advised of all possible side effects before giving essential oils to your
baby, no matter how small the concentrations.
Conclusion
If you are sure your infant is suffering from colic, there are concrete
steps you can take to relieve the symptoms of your colicky baby. A variety
of therapies include changing baby's environment and diet to direct internal
treatment. As always, consult your pediatrician about the range of
alternative treatments available. There are safe, natural and effective
alternatives to pharmaceuticals and artificial chemicals. Homeopathy has
been used for over 200 years to safely treat a variety of nervous and
digestive disorders including colic.
Remember that babies are just getting accustomed to this world and, as they
grow older, their colic will eventually subside. Their digestive systems
will learn how to function well. In the first three months of life, babies

What Is Silent Reflux?
Have you ever heard of silent reflux and believe your child is suffering
from it? Basically, silent reflux occurs when your baby is actually spitting
up but instead of the spit up leaving the mouth it is actually swallowed by
the baby. Many parents thinks this sounds like the perfect solution to
reflux because it is not messy. However, it is not the perfect solution at
all and in fact it is actually a very dangerous condition when it is
untreated. To make things worse, it is hard for doctors to confirm silent
reflux is occurring, but if you think your baby has this condition you need
to be persistent and until the doctor acknowledges and diagnoses your baby’
s condition. Without a diagnosis your baby won’t get treatment that he/she
needs to feel better.
Most pediatricians are accustomed to seeing certain side effects and
symptoms that accompany reflux. However, silent reflux does not frequently
have these symptoms and as a result doctors have a hard time diagnosing what
is going on with your baby. When doctors see raw throats, hiccups, lots of
vomiting and the like it is easy to diagnose reflux, however without these
symptoms present it is hard to determine what is wrong with your baby. Two
of the typical reflux symptoms are weight loss and vomit, however silent
reflux sufferers generally eat more to soothe their pain and do not vomit so
the key symptoms are not present.
If your baby suddenly begins crying during mealtime or even afterwards yet
does not throw up or show any other reason for crying out then perhaps he is
affected with silent reflux. The reason this condition is so painful is
that the stomach acid rises into your baby’s throat burning all the way and
is not expelled in vomit but instead is carried back down the throat to the
stomach burning even more. This generally happens multiples times each day
and obviously your baby’s throat will became raw and sore in no time.
Another sign your baby might have silent reflux is if your baby has bad
breath from vomiting and swallowing it without your notice, gags or chokes
frequently, or burps more often than is normal.
If you believe your baby is suffering from any of the above symptoms then
you should make an appointment with your pediatrician immediately. There are
a couple of ways to change your daily habits and lifestyle that might help
your baby’s silent reflux. Changes like putting rice cereal in formula to
thicken it up, feeding less food more often, or simply changing formula just
might be enough to make your baby feel better and suffer less.
Many times doctors won’t diagnose something they can’t see, but help the
doctor and your baby out by recording how often your baby experiences silent
reflux symptoms and what they are. If you have several weeks worth of
silent reflux symptoms recorded then your doctor just might give your baby
another evaluation to see if silent reflux really is the problem.

全母乳宝宝,四个月的时候出现reflux症状,当时我不知道是reflux(新妈妈经验少啊
),具体表现为:常常archback,不好好吃奶(本来是吃得挺好的)甚至拒绝妈妈的
nipple(其实小人一直很喜欢胸喂的),一抱着靠近胸前就大哭,凑近就扭头,就算勉
强吃点奶,不一会儿就能听见有东西上涌的声音,但是没有吐出来,又很快咽回去了。
白天外婆瓶喂的时候稍好些,但是也比原来吃得少,外婆被小人弄得精疲力尽,跟我说
“就差没跪下来求她喝了”,估计也就是外婆知道这样做没用,否则早就跪了。
这样大概持续了两个礼拜,宝宝每天的奶量从原来的30 oz左右降到15 oz不到,我一到
喂奶的时候就以泪洗面,又气又急,心疼得要命可是又无能为力的感觉。我甚至觉得她
literally很恨我,虽然理智告诉我这根本不可能,但是小东西的nursing strike让我
这个新妈妈感受到从没有过的frustration。上班的时候一个同事随口问起我宝宝的近
况,我都不争气地掉泪(好在人家也是两个孩子的妈妈,才没有被我吓到)。我想那两
个礼拜大概我和她外婆都被她搞得有点babyblue了。
我看这样下去根本就不行,就带着宝宝去看了ped。说到这里,真要感谢她的ped,她听
我描述了宝宝的症状后,马上量了体重身高,发现宝宝原来平稳的percentile曲线一下
子drop down下来了,有很明显的下降趋势——那是当然了,只能吃奶的小人儿每天奶
量不到她应该摄入的一半,怎么可能长重长高呢?她refer我们第二天就给宝宝做个
reflux的检查,看看是不是reflux以及确认情况有多严重。
我们第二天就带着宝宝去了lab,宝宝挺遭罪的,之前要fast4个小时,到了之后喝一种
白色的glucose(据说是一种糖水),然后被扒得只剩diaper,放在一张台子上,一个
specialist就从仪器里观察宝宝胃里的白色glucose返回到喉咙的情况。好在检查的结
果是宝宝并没有表现出明显的reflux症状。
带着疑问回到ped那里,ped说虽然这样,最好还是人为干预一下。因为我家宝宝的身高
体重percentile本来就不高,两样都在50%以下,如果任由她自己outgrow的话,可能会
影响她的正常发育,甚至会miss掉6个月时通常会出现的growth spurt。她跟我们说,
本来可以先开baby zantac吃吃看的,但是因为宝宝的nursing strike已经持续了两个
礼拜,而且现在一天的奶量已经降到了10 oz左右,她担心baby zantac不见得能立刻见
效,而到时候又换药,又要耽误一段时间。所以她说想要直接上prevacid,不过保险不
一定会cover(这两种药的差别和保险cover的问题下面还会讲到)。
我们接受了她的建议,开始吃prevacid,吃药的第二天就有明显好转,一直nursing
strike的宝宝第一次愿意直接吃我了,我当时兴奋得又流泪了,那种感觉就像捧着失而
复得的珍宝!就这样吃了一个月(一次prescription的量),然后回去复诊,ped说要
不我们还是换回去baby zantac吧,温和些,而且保险也cover。结果回家试了一个礼拜
,宝宝似乎又开始nursing strike了,我赶紧给ped打电话,她就又把药换回了
prevacid。这样又吃了一个月的prevacid,再去复诊的时候,ped建议停药试试看。幸
运的是,这时候宝宝已经outgrow了reflux,后来再也不需要吃药就能好好吃奶了。
说心里话,我觉得我们这个ped还是很果断的,现在回想起来还是很感激她当时的建议。
什么是reflux?
Reflux其实是一个简称,全称是Gastroesophageal Reflux Disease,缩写为GERD,中
文译为胃食管反流,宝宝的贲门,也就是胃最上部跟食道连接的部分,因为括约肌没有
发育完全(别害怕,正常现象哈,这是小宝宝消化系统的特性),无法阻止胃里的奶水
倒流,而反流的奶水混合着胃酸经过食道回到喉部的时候,胃酸就会对食道产生烧灼感
,造成宝宝不舒服。其实大人也有这样的情况,特别是准妈妈,就是我们常常说的烧心
(heartburn),可以用zantac缓解。
大部分宝宝都会吐奶,但是如果反流情况严重,有些宝宝就会因为疼痛失去食欲,出现
nursing strike(参考上面说的我家小雅)。其实reflux对宝宝和妈妈的心理健康造成
的危害可能是更严重的,本来因为喂奶已经精疲力尽的妈妈会出现强烈的挫折感,以及
无法帮助宝宝减轻不适的失败感)。所以一旦宝宝吐奶频繁,脾气暴躁,不好好吃奶,
就需要考虑是不是有reflux了。
怎样判断宝宝有reflux?
reflux最常见于2-4个月大的宝宝。典型症状其实上面已经多少提到了一些,有吐奶频
繁(happy spitter不算哈,吐奶之后看上去宝宝很难受才算)、易怒、经常archback
、不容易入睡、拒绝喂奶(nursing strike)等等,尤其当这些不适影响到了宝宝的
growth,就需要引起妈妈的特别注意了。
虽然reflux最典型的症状是吐奶,但还有一种silent reflux更要注意(同样参考我家
小雅)。就是基本不吐奶,但是常常听见很明显的干呕声,还有吞咽声,其实这是反流
的奶带着胃酸经过食道回到了喉咙口,马上又被宝宝吞回去了——我猜这种reflux更难
受,因为食道被胃酸burn了两遍。
而且silent reflux不容易注意到,妈妈们更要观察仔细。
reflux的常见治疗:
reflux其实不算一种病,只是小宝宝的胃肠功能发育过程中的小小不适,而且再怎么严
重的reflux,等宝宝长到一岁左右也会自然消失。但是如果这种不适影响到了宝宝的成
长(poor weight gain or even weight loss),就需要人为干预一下了。事实上,如
果妈妈怀疑宝宝有reflux影响到体重的增长,就应该带宝宝去看ped,ped会检查到宝宝
的体重增长是否异常。还拿小雅为例,她在出现reflux之前的体重是缓慢增长的,从最
开始的15%到25%到50%,但是nursing strike两个礼拜后,一下子又drop到25%,所以
ped才决定开药。
通常ped开的药有两种:baby zantac和prevacid,这两种都是中和胃酸的药(所以说只
能起到缓解不适的作用,并不能治疗reflux——也就是说,reflux是不能靠药物治好的
,只能靠小宝宝自己outgrow)。baby zantac比较mild,就是我们能从药房买到的
zantac的baby version,因为是给小宝宝吃的,所以虽然是OTC,药房里还是买不到,
得要ped开方子才行。通常是液体状,用滴管喂就好了。
prevacid比较strong,是处方药。有的妈妈告诉我说ped开的是片剂,所以要拿回家压
碎了给小宝宝吃,第一不方便,第二小宝宝也不喜欢这种喂药的方式。我们这个ped是
让药房给兑成液体状,所以我们是拿了个带滴管的瓶子回来,喂了一个月,每天一次,
每次喂之前要摇匀。如果可以选择的话,建议妈妈跟ped要求开成液体状的药回来吃。
中和胃酸的药理想的情况下一个月就能看到效果,所以一般ped会先开baby zantac看一
下宝宝有没有好转(恢复正常的weight gain),一个月后复查如果还是不行才开
prevacid。如果是这样的情况,保险公司通常会cover,但是像我们家这样ped给直接上
prevacid的,保险就很难cover了。ped让我自己去跟保险公司argue,我看看两个月的
药钱也就300刀不到,就懒得打电话了。幸运的是,小雅吃了药很快就见到了成效,而
且只吃了两个月就outgrow reflux了。
可以帮宝宝缓解reflux症状的一些非医疗性辅助措施:
我这里说的都是针对母乳宝宝哈,事实上,母乳宝宝出现reflux的可能性比非母乳宝宝
要小,母乳消化快,在胃里存留时间短,而且就算回流,也可能比配方奶要less
irritating,所以,母乳喂养(特别指胸喂)本身也是很好的缓解reflux的措施之一。
研究表明,一半以上母乳宝宝出现reflux的原因跟cow's milk allergy有关。如果喂奶
的妈妈在吃奶制品的,建议停掉一段时间看看宝宝有没有好转的迹象。我家这个碰巧就
是那一半中的一个,而我直到她5个多月大了才发现她对milk过敏,我停掉奶制品一个
礼拜之后,她脸上的湿疹完全消失了,到6个月的时候也不再有reflux了。现在想想,
如果能够早一点发现该多好!
另外,reflux宝宝最好的喂奶姿势是半直立式或坐式,保持宝宝上半身upright,喂完
奶半小时之内不要让宝宝剧烈运动(比如tummy time就最好不要)。喂奶不要喂得过饱
,少量多餐也可以缓解reflux。
对于特别fuzzy的宝宝,妈妈喂奶的时候可以采取skin-to-skin的方式,通常这种零距
离的肌肤接触可以起到安抚宝宝的作用。
reflux宝宝的睡姿:reflux宝宝是最不喜欢lie flat on the back的,这种睡姿下
reflux最严重。妈妈可以在宝宝的crib的mattress下面一边垫本厚点的书(我们那时候
就用了她爹的一本厚厚的finance的书,呵呵),这样人为地把mattress弄出一个倾斜
的小角度,我们家这个就趴在上面睡觉。以前看到TRU有卖那种wedge的,也是可以垫在
mattress下面,妈妈们可以留意一下。
对于出现严重nursing strike的宝宝,可以喂迷糊奶(temporarily),我们家这个就
喂了一阵,不要担心是不是宝宝以后会一直这样下去——他们只是现在暂时不舒服而已
,outgrow了就好了。还要注意的是,喂奶的妈妈一定要保持积极的态度,宝宝拒绝吃
奶的时候也要记得把奶pump出来,好保持产量,千万不要因为宝宝暂时的strike而导致
产量下降,否则一旦宝宝reflux好转胃口大开的时候,妈妈就难以供应宝宝的需要了。

婴儿胃食管反流症状
婴儿胃食管反流并不常见,仅3%的婴儿会出现胃食管反流。如果你的宝宝吐奶很多
,你可能会怀疑他是胃食管反流。但这也要看具体情况。
胃食管反流通常是食管括约肌薄弱或功能不稳定的结果,食管括约肌就是连接食管
(食道)和胃的一个瓣膜。对于一个婴儿来说,如果他每次吃完奶后都会没有明显原因
或毫无预兆地随着打嗝吐出一些东西是正常的,甚至偶尔把喂进去的东西都吐出来也是
正常的。这是婴儿消化系统的特性。
但是,如果你的宝宝很难受,每几分钟就要吐一次,或者经常一天呕吐超过两次以
上,那么他可能患有胃食管反流。胃食管反流就是胃内正在消化的食物经常性上涌,它
会使宝宝脾气变得焦躁,不好好吃东西。在最严重的情况下,还有可能发生气喘、咳嗽
、呕吐。如果你的宝宝有胃食管反流,你一定会发现的,因为你可能会因为他的小脾气
和哇哇大叫早就受不了了。
随着食管括约肌发育得更强健,大部分患有胃食管反流的宝宝在1岁之内会自己好
起来的。但这并不意味着,你可以轻视胃食管反流。胃酸反流进入食管(成年人称为“
烧心”)会使喉咙受伤,留下疤痕,如果胃食管反流情况严重,甚至会导致吞咽困难。
一些宝宝因为疼痛甚至失去食欲。
此外,胃食管反流对你和宝宝的心理健康所造成的危害可能是最为严重的,即使这
只是暂时的。正常情况下,你已经需要每个晚上起床2~3次照料宝宝,想想如果你需要
每天晚上起床5~6次来安慰一个脾气不好、痛苦不安的孩子,并给他清理干净,你能坚
持多长时间呢?
婴儿胃食管反流的治疗方法
你在给宝宝喂奶时,要尽量竖抱,在喂奶过程中或喂奶后,轻轻地给宝宝拍嗝。如
果可以的话,减少每次喂母乳或配方奶的奶量,可能也会有所帮助。你可以通过增加喂
奶次数来补偿每次奶量的减少。
对于配方奶喂养的孩子,你也可以尝试在食物中加一些婴儿米粉,或者试着吃那些
已经混合了米粉的婴儿食品。但在这么做之前,最好咨询儿科医生是否需要尝试这种方
法,以及如何添加米粉。虽然所有这些措施都不能保证能让宝宝舒服一些,有时这些措
施甚至可能会使宝宝脾气变得更不好,但是在带宝宝看医生前,你至少可以尝试这些方
法。
如果你认为宝宝患了胃食管反流,应该带他去看医生。儿科医生会告诉你宝宝不是
真的有胃食管反流,同时,他也能帮助你减轻宝宝的痛苦。一开始,医生可能会给宝宝
开些药。抗酸药或酸阻滞剂对于有些胃食管反流的宝宝会立即起效,大部分宝宝也能够
耐受儿童剂型的碳酸钙制剂等抗酸药,以及小剂量的酸抑制剂,比如西咪替丁和雷尼替
丁等,但是要注意,在咨询医生以前,你不能自行给宝宝服这些药。
理想情况下,服药后一个月内宝宝的胃食管反流症状就能改善。另外,胃食管反流
的宝宝也可以服用促进食物沿消化道向下蠕动的药物。如果宝宝是典型的胃食管反流,
医生应该能够找到一种解决胃食管反流的、没有副作用的治疗方法。
如果药物无济于事,宝宝还是觉得很难受,医生可能会做进一步检查,来确认到底
是不是胃食管反流造成的。胃食管反流的诊断性检查包括上消化道造影以及24小时食管
pH(酸碱度)测定。
医生会用一根很细的管从宝宝的嘴里伸到食管底部,监测24小时,这个检查可能会
使宝宝感到很紧张,并且不舒服,但这是最可靠的检查方法,对胃食管反流的宝宝来说
,有时候也是必需的。

抱抱mm和宝宝,一个小时,宝宝睡的太少了。应该是哪里不舒服,多抱抱他吧。吃完奶
