应该填什么？ H1b approval notice上的I94的签证起始日期还是最近一次(当时还是F1
签证）真正过边境的日期？
2. Part3. 1a: I request that my current or requested status be extended
untill:(mm/dd/yy) 我LP的H1b申请刚递交（拿到了receipt number),还不知道批准的
日期到什么时候这儿怎么填？

入境日期
你得选H1有效期。下面填h1 receipt number

还有一个问题是我最近一次入境时还是F1身份，还需要把当时的材料（签证，I94)递交
吗？

一般是把从最近一次入境到现在所有的身份文件附上。包括I20，I94，签证。
