女儿今年七岁。自小就发现她的下齿包裹上齿。现在上下幼齿还有几棵没有完全换。最
近CONSULT了三个整牙医生。
有两个认为应do it ASAP, since the bottom teeth will interfere with the
development of the upper jaw.
But the other doctor (with 35 years of experience) believed we should do it
next year when she is eight years old. He thought it was just the teeth
problem, not the structural (jaw) problem. And he did not think the
development of the upper jaw will be impacted. Only by then (next year ,
after she gets most of the permanent teeth), it is possible to correct that
problem. Otherwise it is over treatment.
We are confused at this point, who should we listen to? Could anyone with
similar experience help please?

如果有好几颗牙地包天，现在就整，下牙床戴一个retainer, 上牙会顺着retainer往外
逐渐滑出来。大致三个月矫正地包天。费用不高。就几百美元。
整得越早越容易。越晚的话就越疼越难，需要越长时间才能校正。
如果只有一两颗牙地包天，那可以等到八九岁再整。

要是等全部幼小齿换完再整牙，就要12岁左右了。那时候就麻烦了，
搞不好还得动手术，一套牙整下来，五千多。又很痛苦。因为永久牙
牙根深，要他们移动会很痛。
乘着现在幼齿还多，把能整的先整了，几乎无痛，把地包天给纠正了，
让上颚无阻碍生长，免得以后上颚crowded, 新换的牙只能歪着长。

你是说上门牙过度向前？我个人经验天包地没事。我就是上门牙完全在下门牙
之前，一点事都没有。不影响吃东西或者发音。
倒是地包天一定需要矫正，
否则upper jaw被下牙挡住，不能正常生长，上牙床的牙齿就只能歪着挤出来。
上面需要比下面更多的地方，因为上门牙比下门牙宽多了。
不过从整牙的角度讲，天包地也是需要整的，上下门牙重合两毫米是最佳状态。
但是这个不用尽早整，8-9岁戴braces矫正即可。

今天喷嚏猛打，果然有人呼唤，呵呵。
根据楼主所述，个人还是认为等孩子8个门牙恒齿张齐后，在做phase1 整形。
呼唤YCC 会诊，呵呵。

我的经历实在太像了。区别就是我儿子已经九岁了。很有经验的医生也是让我们等一年
，年轻的让我们立即作。准备再去看一个，也是很有经验的。
你们最后怎么决定的？

这个没有看到小朋友很难说啊。现在前面门牙已经换几颗了？这个地保天是dental
problem还是skeletal problem, 有没有CR/CO shift等等检查都会影响最后决定。但一
般来讲，地包天，unilateral shift都是有可能影响上下鄂发育的问题，需要尽早治疗
的。另外牙医也不是越老越好的，现在技术，知识更新变化很快，不是谁都愿意经常学
习的，特别一些岁数大大，快准备退休的牙医。我个人认为一些毕业行医十多年的医生
更好一些，当然这不是绝对的。

女儿上面换了两颗门牙，下面三四颗。我们主要想搞清楚究竟会不会对上鄂发育有影响
？我们现在打算先去做个X-RAY以确定是否skeletal problem （虽然第三个医师主观认
为不是）。
前面两个医生PHASE ONE 的估价都是三千多，怎么这么贵？他们说只是睡觉戴个
DEVICE撑一撑，并非真正整牙（毕竟牙还没换完）。

牙医推荐的那个撑一撑的是expander了。确实是比较贵的，功能在把上颚撑大，
我说的 retainer不知道你看的整形师为什么不推荐给你，又便宜，又好用。
而且只要把上牙滑出来，上颚生长就不会被下颚阻挡了。滑出来之后，上颚
也明显被撑大一点。或许都可以不用expander了。
不过也可能是这个原因，整形师就不推荐了，免得赚了几百刀，丢了几千刀。
要不你再打听详细一些？
我的理解是phase one包括expander + braces,
有些孩子上下颚发育良好，就可以不用expander, 只用braces,可以便宜很多。
phase two是12岁以后，所有牙齿基本都换完以后。要戴braces弄弄齐整，
纠正咬合不正。

Lots of information, thank you so much!
We are making appointment to take the full mouth X-Ray at my dentist office
(100% cover for every five years; if do it at Orthodontist, only 50% cover).
Then we'll talk to them for the detail before we decide where to go.

奥，原来你们还没有做全嘴X光。
有些整形医师很nice, 为了招揽顾客，可以免费做x光之后再给出estimation.
那就可以做出比较可靠的推荐了。
因为上颚是否过于拥挤，没有全嘴X光是看不出来的。
你先去牙医那里做也是好的，最好要一次成形的那种panorama的，而不是照几个区域再
拼凑起来的。

I believe what LZ's orthodontist talked about is face mask although it is
often used with expander together. If LZ's kid has skeletal problem (
retrusive upper jaw), what you talked about won't help much. Face mask will
help upper jaw growth to catch up with the growth of lower jaw.
BTW: the x ray can help to evaluate situation is cephalometric x ray and
panoramic x ray, not the full mouth x ray. Please double check before taking
the x ray.

我儿子也是下门牙包住上门牙，牙医说没有必要太早矫正。因为他的后面的大牙咬合正
常，所以不会影响上下颌骨的发育。

Thanks!
The dentist recommended the same thing. So we decided to go to the
orthodontist for x-ray, even though it cost $250.
