It should be fine. My I797 also only contains my name, but in the first paragraph of the small characters in the front page it mentions "your immediate relatives are waviered too"

IRB means institutional review board. For most universities, they have their own IRB. And it often is mandatory to submit protocol to University IRB for them to review. This often is quite annoying especially in large multi-center clinical trials, as a center IRB will be ideal.
NPR this noon talked that the hela cell was taken from the black lady without her knowledge and her family found out that 25 years after her death . it is a nice coincidence.
You guys had major violations and should be suspended. It does not matter how many pages you have for the protocol, it matters if you have all the information available and considerations for human subject protection. A potential conflict exists when a subject's participation in the research to benefit mankind may place him/her at personal risk of harm. I did some research on TMS for my qualifying exam. My boss did a lot of work in Alberta. Mostly on muscle properties after reinnervation. The last 10 years she worked on peripheral nerve stem cell transplantation and stimulation. AAALAC is coming next month. We are working tirelessly on the paperwork.
