医生自然就把ID number 填成了driver license number，现在有些后悔，当天应该记
着带护照的，现在这样的话会不会有麻烦？485材料附上driver license的copy会不会

那个表格写的很清楚，你可以选择是什么证件
我那个医生注明了是驾照，然后写号码
没问题的
