Full copy of passport and any expired passports (every page - even if it
blank!)
Clear copy of the I-94 card (front & back) issued upon last entry into
the U.S.
2 color copies of one additional piece of picture ID (driver's license,
state ID etc.)
Copies of all previous H-1B visa approval notices, work authorization
cards, and student I-20s (we must have ALL of the paperwork to show the CIS
that you have always been in status).
Copy of birth certificate (this must be registered with the proper
government body i.e. municipality) plus a translation if applicable.
Copy of marriage certificate with translation, if applicable
Medical exam at CIS designated location. Please note that the medical
exam is time sensitive; therefore you should wait until your priority date
is current to schedule the exam. However, you will need to bring proof of
your vaccinations (which can be completed ahead of time) to the medical exam
. Enclosed please find the list of CIS vaccination requirements.
We will need standard size copies of all documents. If any family members (
your spouse or children under 21 who are not U.S. citizens and are currently
living in the USA) will be applying with you, we will need each of the
above items for each applicant.
The following information is very important:
If you have ever worked without CIS authorization or remained in the U.S
. after the expiration of your I-94 card or visa, you may need to pay a $1,
000.00 penalty fee to the CIS. If you are concerned that this may apply to
your situation, please discuss this matter with your attorney.
You will need to remain employed full time with your current employer (
the sponsor of your I-140 petition) for at least 180 days after filing the I
-485.
If your address or phone number changes, please inform us at your
earliest convenience. Keeping us updated will allow us to contact you
immediately once your priority date becomes current.
