employment verification letter
materials that prove the qualification as a petitioner (if total employee <
100, they may need to show tax and revenue forms)
Some columns in 140 form, e.g., tax EIN, gross income, etc. You may need to
ask them to fill. Take a look at the 140 form.
Checks for application fee.

Thank you very much for your reply.
I just read something saying that the I-140 for EB1B has to be filed by the
employer or the attorney. Is it true?
I am currently working in a research center which is affiliated with a
University and my title is a permanent one in. The problem is that this
research center has been downsized from ~300 persons to ~200 persons in the
past couple of years and there was no personnel handling the documents.
However, the University insists working with the our research center only.
Under such circumstances, is it still possible to do EB1B DIY?

u r right. it should be the employer or your attorney to file your case.
just ask your hr, if they can allow u to prepare your package. sometimes
they insist using their own lawyer.
