大名鼎鼎的New England Journal of Medicine在最近一期上发了一个对针灸的review
，基本否定了针灸的治疗效果,同安慰剂组没有区别：

这个相当有意义了，因为有没有效果是统一的标准，这个相当于是对针灸的严格临床试
验，实验结果证明没有治疗效果，你用针灸和用安慰剂的结果是一样的...
对于有没有效果这个事，不存在所谓“体系”不同的问题，除非中医能否定西医的临床
试验的科学性，或者能提出自己的临床试验方法。

本着科学的态度，你应该写清楚这次结果针对的是chronic lower back pain. 那些做
实验的病人真可怜。让我想起了俺们家附近的癌症研究所，用各种fancy的统计工具，
算那种新药对癌症管用。人survive了几年,就是他们公式里面的一个argument.

那些做实验的病人可怜主要还是因为得了那个病可怜，　要是还有更好的办法，　人家
也不会去那个地方，　要是这些药真能够治好他们或治疗好其中一部分，　那是那部分
人的福了．
当然，　他们也可以象林妹妹一样，　去赌另一条路，　不过赌赢的机会并不会更大就
是了，　如果赌输了，　还是一样的可怜．
那个针灸实验，　我最怀疑的是他们哪里去找那么多合格的针灸大师来做这实验，　要
是找些那种能扎出血包不给人换针的好中医，　这实验结果可想而知．

对于癌症的临床实验:
1. 新药的临床试验的对照组通常不用安慰剂, 而用常规使用的抗癌药; 因为在这种情
况下用安慰剂不太合理.
2. 但是, 安慰剂作为对照住仍然是可以(少数临床实验如此), 在这种情况下, 安慰剂
组的病人最终会得到治疗.

那篇文章是收费的。LZ只是要给你看个结果，不是很care是怎么弄出来的。我怀疑这后
面是什么人在推动. chronic lower back pain这种毛病死不了人，但是会拖时间很长
。让我想起一个笑话，一个医生告诉他的病人，我有一个好消息和一个坏消息。坏消息
是你得了什么什么病，要看很长时间。 好消息是我儿子念大学的学费有着落了.

Anybody actually read this article before postted comments here?
Tons of evidence showed acupuncture did reduce pain for pt with chronic back
pain, at least acutely, both real and sham. No doubt about it.
I am not a Chinese medicine fan, but I would love to know the fact.

这是说给不搞生物或医学科研的人听的，怕人家以为是哪个地方的非专业的传媒小报的
报道。跟科学的“讲求实证、逻辑，反对权威主观论断”无关。

原文的附件（ http://www.nejm.org/doi/suppl/10.1056/NEJMct0806114/suppl_file/nejmct0806114_appendix.pdf）里有关于安慰剂组的描述， 在2006,2007,2009的三个共近2千人的临床试验里，患者被分为三组：实验组、安慰剂组，其他对照组。2006年的实验里，一个147人的实验组按穴位扎，一个75人的安慰剂组是胡乱扎，一个79人的对照组不扎针，这个实验的结果显示：按穴位扎和胡乱扎没有区别（P=0.26)；扎和不扎有区别(P<0.001). 2009年的实验里，实验组还被细分为individulized和standardized两组，结果类似。

文章的结论是针灸扎和胡乱扎一样效果，扎比不扎好，呵呵。所以结果就是针刺可以镇
痛，跟针灸没关系...

A large trial in Germany,50 3093 patients with chronic low back pain for a
mean of 7 years were randomly assigned to receive either acupuncture or no
acupuncture in addition to usual medical care. The primary end point was
back function, as assessed with the use of the Hannover Functional Ability
Questionnaire, which generates a score ranging from 0 to 100, with 100
representing perfect back function. At 3 months, the mean back-function
score in the acupuncture group had increased from 61.8 to 7

这个听起来有点道理，记得针灸镇痛部分原因就是靠针刺后内啡肽释放什么的。应该再
做个实验，用针刺以外的方法让病人有疼痛感，然后看看背疼好些了没有。

看了下这个德国试验的原paper，这个paper得出的结果同其他paper是相同的，它比较
的是针灸组和什么都不扎的对照组（这个实验里面没有安慰剂组)，同其他文章得出的
结果一样，扎比不扎强，没比较扎穴位和胡乱扎，呵呵。
