I-140, the first step in green card application to get the immigration
visa quota. This is the most critical step.
I-485, the adjustment of status to get the green card finally.
I-907, premium processing to speed up the I-140 processing to be within
15 days, which applies to RFE response, if any. It is the same form for
H1B expedition, as well.
I-765, EAD (for employment authorization), free with I-485 application,
or while 485-pending.
I-131, AP (advanced parole), the travel document to enable the applicant
to go travel outside of US without abandoning his/her immigration
application, free with I-485 application, or while 485-pending.
I-693, report of medical exam and vaccines. It is needed for applying
for 485. It is in a sealed envelope from USCIS designated doctors.
I-130, similar to I-140, for family based immigration.
G-325a, biographic information for people between 14 to 79 years of age,
for I-485 application.
G-1145, email or text-message consent form, to request the notice being
sent to your email or phone.
AR-11, change of address form, in case you move any time during your
application.
G-28, if you are using a lawyer, the petitioner (you, if EB1A or NIW, or
your company, if EB1B) grants the lawyer the full right to handle your
case and do anything on behalf of the petitioner.

Current (10/6/2011 and in foreseeable future) fee structure:
I-140, $580. Only the main petitioner needs to file this.
I-907, $1225.
I-485, $1070 ($985 plus $85 biometric fee for finger printing). If it's
a couple who are applying for the green cards together, each of the
couple needs a separate application, namely, double the fee. Both
applications can be put within one single envelope though.

Regarding premium processing, nearly all categories can be expedited,
except NIW.

However, for Chinese, because of 排期, only EB1 is worth PP.

Timing-wise, things vary, especially for a regular I-140.
The quickest I-140 case I saw on this board took only 12 days without
PP, and that case might have been processed as if it was a PP case. The
longest case I saw was nearly over a year, that's very unlucky perhaps
due to piled up applications in TSC. However, generally, regular I-140
may take 4-6 months, and if there was any RFE, the response time could
be from a month to 3 months, and the decision time after the IOs receive
the RFE packages could be another one or three months.
I-485 often takes 3-6 months. Typically after 4 months or whatever date
that is announced on USCIS website as the processed date, you can do
service request.
EAD and AP processing is independent from the I-485 processing, and they
can be approved before the fingerprints are taken, as the fingerprints
are only required for I-485 approval. EAD and AP can be a combo card, or
two separate cards. EAD and AP often take three months to be approved.
If you need an AP for emergent travel, you can walk into a local office
to request urgent AP, which may have an earlier expiration date than a
regular AP.
