A doctor treating a patient with Gastroesophageal Reflux Disease (GERD)
Sean - Good evening all. It’s time for our weekly Hansons oral English lecture yet again. I hope you are all ready and raring to go. Let’s begin.
D: Hi Miss Paton. Please sit down and fill me in on what's bothering you.
P: Hello Doctor. Recently I've been experiencing severe heartburn. It's been occurring at least 4 day's per week for around 2 months now. I've tried heartburn relief tablets, and even stopped smoking 3 week's ago but nothing seems to be helping me.
D: I think it may be possible that you have Gastroesophageal Reflux Disease, also known as GERD. I'm going to ask some more questions now to find out what our next steps should be.
Can you tell me if your diet contains any of these foods: greasy foods, spicy foods, chocolate, peppermint, coffee, foods containing tomato products, or alcoholic drinks?
These are all foods which have been linked with causing GERD in some cases.
P: Well Doctor, I'm embarrassed to say that all of these food types are in my diet, but not everyday and not in excessive amounts when I do consume them. So you think this could be the cause of this GERD that you have mentioned?
D: I think it could definitely be helping to influence these symptoms, but you said you do not consume these products all the time so I think there is reason to believe that something else could be included with causing these symptoms you are feeling.
My opinion is that I recommend that you go and see a specialist in this field called a Gastroenterologist. They will be able to run certain tests which can identify your problems cause and then rectify it.
P: Okay Doctor. What type of tests will they run?
D: They have a varied range of tests available. They include: Esophageal pH and impedance monitoring, Upper gastrointestinal (GI) endoscope, Upper GI series, Esophageal manometry, Bravo wireless esophageal pH monitoring as well.
P: That's a lot of tests, so i feel confident at least one of them can pinpoint my problem and help us fix it. What will happen after the tests are completed?
D: After the tests are completed they will then discern what medications you should use. These medications are Proton pump inhibitors, which are one of the main pharmaceutical treatment options for people with GERD. They decrease the amount of acid produced by the stomach. Other options include: H2 blockers, Antacids, Prokinetics, and Erythromycin. A change in lifestyle is also recommended. More exercise is very important.
P: And what if the medications and lifestyle changes do not work?
