A doctor helping a patient who has gallstones.
Sean: Hi everyone. I hope you are all well. It’s time for this weeks lesson to begin. Here we go.
D: So Mr Anderson, you are experiencing abdominal pains for prolonged periods of time, and quite often your temperature is above 38°C, is that correct?
P: Yes doctor, that is correct. I really don't know what's wrong. Hopefully you can shed some light on this problem.
D: I think you may have a problem in your gallbladder, possibly gallstones, but I'll need to do tests to confirm. Right now, I'm going to place my hand on the upper-right part of your tummy, when I do this can you breathe in and tell me if you feel a pain or not?
P: Okay doctor.
D: Ready, breathe in now.
P: Yes doctor, it's painful.
D: Well, it's as I suspected. Your gallbladder is inflamed, probably due to gallstones. I'm going to run a blood test to check for infections or to check if your liver is working properly.
Once the test results are back we will know more about how severe this issue is. It is possible you may need to stay here at the hospital to have further tests immediately, so sit tight and I'll get the results ASAP.
P: What types of tests might be needed?
D: If the results show signs of gallstones then we may use an ultrasound scan, an MRI scan, a Cholangiography or a CT scan to find out more information.
P: Okay, what does the treatment for gallstones entail?
D: It depends on how the symptoms are affecting your daily life. If you don't have any symptoms, active monitoring is often recommended. This means you won't receive immediate treatment, but you should let your GP know if you notice any symptoms.
As discussed, you have abdominal pains frequently so we will assess how the pain affects your daily life, and prescribe painkillers to help you, but if you think the pain is too much then surgery to remove the gallbladder is usually recommended.
The gallbladder isn't an essential organ and you can lead a normal life without one.
P: Oh my, a possibility of surgery. What is the procedure called?
D: We have different options available, 1 is called keyhole surgery (laparoscopic cholecystectomy), and the others are Single-incision keyhole surgery, open surgery, Endoscopic retrograde cholangio-pancreatography (ERCP) and we also have medication to dissolve gallstones.
